Nonconformity and corrective action is how a certified organization handles what went wrong and makes sure it does not happen again. When the process drifts, findings get closed with a correction, the cause survives, and the same issue comes back under a new report number.

What the Nonconformity and Corrective Action Maturity Check measures
The check scores eight elements that every standard shares, then scores each standard you select on its own requirements.
- Reporting and intake: whether nonconformities, complaints and incidents reach the process from every source.
- Immediate reaction and extent: whether the problem is contained and its reach is checked before anything else.
- Severity and routing: whether each finding is routed on its effect rather than handled the same way every time.
- Correction versus corrective action: whether fixing the instance is kept separate from removing the cause.
- Cause analysis: whether investigations reach a cause that explains the finding.
- Actions and ownership: whether each action has an owner, a date and a defined result.
- Effectiveness: whether effectiveness is checked against evidence rather than a signature.
- The improvement loop: whether trends from corrective actions feed back into risk, objectives and management review.
The standard-specific questions cover what only that standard requires. For example, ISO 45001 requires worker participation in the cause evaluation and actions that follow the hierarchy of controls. ISO 13485 requires verification that a corrective action has not compromised regulatory compliance or device safety.
Clauses covered: ISO 9001, ISO 14001:2026 and ISO 45001 clause 10.2, ISO 13485 clauses 8.3 and 8.5.2, and the nonconformity requirements of ISO 7101.

How the score works: four maturity bands
- Exposed (0 to 29). The process exists on paper more than in practice. Findings are handled, but causes survive, which is why the same conditions keep producing them.
- Documented (30 to 58). A documented process people follow. Causes are recorded, but the evidence that removing them worked is thin. This is where most certified organizations sit.
- Controlled (59 to 80). A well-implemented certified system. Findings are routed on effect, causes are pursued properly, and effectiveness is checked against evidence. This is a legitimate place to rest.
- Measured or better (81 to 100). The process is working, measured and adjusted from what the measurements show.
What to fix first
Improve one element at a time. Attempting to move every element at once is the most reliable way to move none of them. Your results list the order to work in, starting with the elements that are clause requirements.
